Tretinoin is Category C in pregnancy and contraindicated by most OBs. Safe alternatives: bakuchiol (plant-based retinol-like effect), azelaic acid (acne + pigment), niacinamide (general anti-aging), vitamin C (brightening + collagen), and SPF 50+ (the most important anti-aging tool of all).

Why tretinoin is off-limits
Topical tretinoin is FDA Category C in pregnancy due to small risk of teratogenic effects. While topical absorption is minimal (under 5%), most OBs recommend stopping during pregnancy, breastfeeding, and ideally 1 month before conception. The risk is small but not zero.
